1. Gather Essential Medical Documents
Step 1: Schedule a Doctor’s Appointment
Taking this step ensures your loved one's safety and well-being! During the visit with your primary care doctor, the goal is to obtain a prescription and a Letter of Medical Necessity (LMN) for the Cubby Bed. Be sure to focus on your child’s specific safety concerns and explain why the Cubby Bed is medically necessary, particularly if other solutions have failed in the past. If you have an occupational or physical therapist (OT/PT), we recommend having them write the LMN. If not, a letter from your primary doctor works just fine.
Step 2: Obtain a Prescription
After discussing your loved one’s needs, request a prescription for a "Cubby Safety Bed." To make things easier, we’ve created a Required Documents Packet. Bring this packet to your appointment or share it with your healthcare provider in advance. It includes detailed instructions and necessary prescription forms. Reviewing it ahead of time is helpful, but your doctor will complete the forms during your visit.
Step 3: Secure a Letter of Medical Necessity (LMN)
The Letter of Medical Necessity (LMN), written by a healthcare provider such as your doctor or OT, will explain why your child needs a Cubby Bed. It should outline specific safety concerns, previous attempts at solutions, and why the Cubby Bed is the most appropriate choice. A detailed guide for preparing LMNs can be found here.
Important: These medical documents must be sent to your Durable Medical Equipment (DME) Supplier. Remember, Cubby Beds is the manufacturer and does not handle insurance billing or document submission, which will be covered in Step 2.
2. Partner with a Medical SupplierStep 1: Work with Us—Your Durable Medical Equipment (DME) SupplierAs your trusted Durable Medical Equipment (DME) Supplier, we are here to assist you throughout the process. We supply the Cubby Bed and work with various insurance providers, including Medicaid.
Our team will help ensure that your documents are ready for submission, and we’ll guide you through each step to make the process as seamless as possible.

Step 2: Finalize Your Documents
Once you’ve identified a DME supplier, they will review all your medical documents to ensure everything is in order for insurance submission. Your supplier can also help confirm the appropriate billing code for your specific insurance plan.
It’s important to maintain communication with your DME, checking in regularly to stay updated on the status of your submission.Verify with your DME if your insurance accepts the Required Insurance Documents mentioned in Step 1, or if additional documentation is needed. Our packet gathers essential information, but ensure that all equipment-specific documentation (such as practitioner’s chart notes, therapist evaluations, or state Medicaid forms) is provided where applicable.
Note: Cubby Beds does not handle insurance billing, so you or your primary care doctor will need to submit the medical documents to your chosen Durable Medical Equipment (DME) Supplier.
3. Submit to Insurance
Step 1: DME Submits Claim to Insurance/Medicaid
Once we have all the necessary information, our team will submit your claim and collaborate directly with your insurance company to obtain approval for your Cubby Bed based on your coverage benefits. This step is crucial for ensuring your loved one's safety and comfort. Keep in mind that the approval process may take some time, depending on your insurance provider, location, and individual circumstances. Be sure to follow up regularly to check the status of your claim.
Step 2: If Approved, We Will Order Your BedGreat news! If your claim is approved, you’re one step closer to providing safer sleep for your loved one.
